Final Workshop Registration
Online registration is preferred. However, if you'd like to mail in your final registration and payment with a check, please complete this online registration and select "Pay by check" instead of entering credit card details. Print out your registration receipt and mail it with a check in U.S. dollars made out to Discalced, Inc.:

Registration deadlines: Final registration and payment, whether online or by mail, must be received in full no later than 30 days before the start date of the workshop. Registrations received after this time may be accepted on a case-by-case basis, but are not guaranteed.

Cancellation policy: Participants who cancel before October 1, 2024 will receive a full refund less a $25 registration fee and $15 cancellation fee ($40 total). Participants who cancel on or after October 1 will not receive a refund, unless we are able to fill your place in the workshop. Participants who test positive for COVID-19 within 5 days of the workshop beginning, and show proof of a positive test, will receive a full refund. Cancellations must be communicated via email to admin@danceforpd.org. In cases where a refund is not possible, credit for attendance at a future Dance for PD workshop will be offered.
All online payments occur through this certified, secure SSL-encrypted shopping cart and are processed by Discalced, Inc (dba Mark Morris Dance Group).
Credits: Attendance at this workshop earns you 20 credits (12 for the On Demand version) that you can apply toward establishing eligibility for or maintaining Dance for PD certification.
Affiliation: Dance for PD is a registered trademarks and may not be used to describe any classes without official licensing. Participation in this workshop is for training purposes only, and does not license you to use these trademarks.
Photo/video release: By registering, I hereby allow Dance for PD, Mark Morris Dance Group, Step Up for Parkinson's, and guest instructors to publish photographs or videos taken of me, and my name, for use in printed Dance for PD publications and websites, as well as in publications and websites published by Dance for PD workshop hosts and presenting partners. In consideration for my voluntary participation in publications produced by Dance for PD, or any collaborator, I agree that I will receive no financial compensation. Furthermore, I agree that participation in any print or online publication produced by Dance for PD, Mark Morris Dance Group, Step Up for Parkinson's, guest instructors, or any outside third party interests, confers no ownership or special rights whatsoever. I release the Mark Morris Dance Group and any outside third party interests for any claims of liability by me in connection with my participation in the production of these photographs.
Waiver: I understand that participating in the workshop may require mental judgment and a high degree of physical fitness, agility, stamina and strength, and that this may include strenuous exercise. I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I may incur as a result of participating in the workshop. I, individually, and on behalf of my heirs, successors, assigns and personal representatives, hereby agree to discharge, release, and hold harmless the Mark Morris Dance Group and Step Up for Parkinson's, their respective board members, trustees, officers, employees, agents and independent contractors, from any and all liability whatsoever for any and all damages, losses, or injuries (including death) I sustain to my person or property or both, including but not limited to any claims, demands, actions, causes of action, judgments, expenses and costs, including attorney’s fees, which arise out of, result from, occurring during, or are connected in any manner with my participation in the workshop.
