Final Workshop Registration
Prerequisite: By registering for this workshop, I acknowledge that I have completed a Dance for PD introductory workshop or have received written approval from the Dance for PD team to participate in this workshop.
Currency: Please note that all purchases are made in US dollars at the closest approximate rate to the quoted registration fee. Slight adjustments due to currency fluctuations are to be expected and are beyond our control.
Registration deadlines: Final registration and payment, whether online or by mail, must be received in full no later than 5 days before the start date of the workshop. Registrations received after this time may be accepted on a case-by-case basis, but are not guaranteed.

Cancellation policy: We understand that plans can change and aim to offer as much flexibility as possible. All in-person registrations canceled on or before August 1 are eligible for a full refund, with no administrative or cancellation fees. Cancellations after August 1 are non-refundable, but you will receive a credit that can be used for another workshop or to purchase any of our online resources or media. Participants who test positive for COVID-19 within five (5) days of the workshop start date, and who provide proof of a positive test, will receive a full refund regardless of date. All cancellations must be communicated in writing via email to [email protected].

Credits: Attendance at this workshop earns you 8 credits that you can apply toward establishing eligibility for or maintaining Dance for PD certification.
Affiliation: Dance for PD is a registered trademarks and may not be used to describe any classes without official licensing. Participation in this workshop is for training purposes only, and does not license you to use these trademarks.
Photo/video release: By registering, I hereby allow Dance for PD, Mark Morris Dance Group, Dance for Parkinson's Australia, The Australian Ballet, and guest instructors to publish photographs or videos taken of me, and my name, for use in printed Dance for PD publications and websites, as well as in publications and websites published by Dance for PD workshop hosts and presenting partners. In consideration for my voluntary participation in publications produced by Dance for PD, or any collaborator, I agree that I will receive no financial compensation. Furthermore, I agree that participation in any print or online publication produced by Dance for PD, Mark Morris Dance Group, Dance for Parkinson's Australia, The Australian Ballet, and guest instructors, or any outside third party interests, confers no ownership or special rights whatsoever. I release the Mark Morris Dance Group and any outside third party interests for any claims of liability by me in connection with my participation in the production of these photographs.
Waiver: I understand that participating in the workshop may require mental judgment and a high degree of physical fitness, agility, stamina and strength, and that this may include strenuous exercise. I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I may incur as a result of participating in the workshop. I, individually, and on behalf of my heirs, successors, assigns and personal representatives, hereby agree to discharge, release, and hold harmless the Dance for PD, Mark Morris Dance Group, Dance for Parkinson's Australia, The Australian Ballet, and guest instructors, their respective board members, trustees, officers, employees, agents and independent contractors, from any and all liability whatsoever for any and all damages, losses, or injuries (including death) I sustain to my person or property or both, including but not limited to any claims, demands, actions, causes of action, judgments, expenses and costs, including attorney’s fees, which arise out of, result from, occurring during, or are connected in any manner with my participation in the workshop.
